DIABETES & SOCIAL DETERMINANTS OF HEALTH

Flip the Pharmacy (FtP) is designed to help community pharmacies transform their practices and build sustainable models of care that allow them to provide expanded access to patient care service in their communities. FtP helps pharmacies build important infrastructure in their practices to create the capacity to deliver these critical patient care services. FtP provides changes packages to keep pharmacies on track to achieve practice transformation. The change packages provide guidance to help pharmacies transform key areas of your pharmacy: your workflow, your patient care processes, and your business model.

The Change Packages provide focused practice transformation activities to develop each of the six domains.

  • Domain 1 : Leveraging the Appointment-Based Model

  • Domain 2 : Improving Patient Follow Up and Monitoring

  • Domain 3 : Developing New Roles for Non-Pharmacist Support Staff

  • Domain 4 : Optimizing the Utilization of Technology

  • Domain 5 : Establishing Working Relationships with other Care Team Members

  • Domain 6 : Developing the Business Model and Expressing Value

Details on each Domain will be expanded on in each of the four progressions

  • Opioid Focus

  • Immunizations

  • Hypertension

  • Diabetes & Social Determinants of Health

Embedded within these change packages are countless resources and best practices solicited from pharmacies across the country. These resources have been extracted and organized in this Content Library for your reference.
